Sub: Revoking of suspension of empanelment of Sanjeevani Hospital (A Unit of Sukhmani Nursing Home Pvt. Ltd.) 765-766, Lakhanpur, Awadhpuri Road, Kanpur.
With reference to the above mentioned subject the undersigned is directed to draw attention to the office order No. 3-2/2011-12/CGHS/KNP/4241-S6 dated 03-09-2015 vide which empanelment of Sanjeevani Hospital (A Unit of Sukhmani Nursing Home Pvt. Ltd.) 765-766, Lakhanpur, Awadhpuri Road, Kanpur was suspended from the list of empanelled Hospital under CGHS, Kanpur for one month or till further orders. It has now been decided to revoke suspension of empanelment of Sanjeevani Hospital (A Unit of Sukhmani Nursing Home Pvt,.Ltd.) 765-766, Lakhanpur, Awadhpuri Road, Kanpur) in public interest with issuance of this order w.e.f. 03.10.2015. The empanelment would be for all those facilities available in the Hospital and as per same terms and conditions on which it was empanelled prior to 03-09-2015.
