Sub: De-empanelment of Usmanpura Imaging Center from the Empanelled HCOs list of CGHS AHMEDADABAD on violation of MOA-18 (1) & (7) -reg.
Usmanpura Imaging Center (A Unit of Pixel Diagnostic), a CGHS, Ahmedabad empanelled HCO has violated MOA 18(1) & (7) despite repeated instructions resulting complaints from pensioners. CGHS is empaneling HCOs for providing services to its beneficiaries including cashless services to pensioners and other eligible beneficiaries. On failure of providing services and credit facilities to eligible beneficiaries, Usmanpura Imaging Center is hereby DE-EMPANELLED from the empanelled HCOs list of CGHS Ahmedabad with effect from 26-08-2021 as per MOA and Advisory to empanelled hospital by Min. of Health & F.W. dtd. 19/8/21.
