OFFICE MEMORANDUM

Subject:- Empanelment of private hosptals (including dental clinics and eye care centres and diagnostic centres under CGHS, Nagpur.

I. The undersigned is directed to invite reference to this Office Memorandum No: CGHS/NP/ Admn/HEMP/ 2013/941 dated 22.07.2013 vide which the list or private hospitals (including dental clinics and eye eare centres) and diagnostic centres empanelled under CGHS Nagpur have been notified under different categories.

2. The undersigned is directed to enclose a further list of hospitals and diagnostic centre in the respective categories mentioned in the application for ”Continuous Empanelment Scheme” and tender documents that have conveyed their acceptance of the CGHS rates notified under CGHS, Nagpur and have signed the Memorandum of Agreement and have also furnished the appropriate Performance Bank Guarantee. Those hospitals and diagnostic centres are also taken as included in the lists of approved hospitals for empanelment under CGHS Nagpur.

It has now been decided that these Hospitals will be eligible to treat CGHS beneficiaries and charge at the CGHS approved rates with effect from the date of memorandum.

3. A Corrigendum showing the correction in telephone number address and additional facilities offered by some empanelled hospitals is also enclosed.

4, The empanelment shall be for a period of one year or till next empanelment, whichever is earlier.

5, This office Memorandum and the rates applicable under CGHS for hospitals and diagnostic centres can be downloaded from the website of CGHS http://cghs.nic.in or http://msotransparent.nic.in/cghsnew/index.asp
