Subject: Updation of NABH Accreditation status of Smile ‘N’ Shine Dental Care Centre, 44, Madhya Marg, DLF PH -2, Gurgaon- 122002, from Non-NABH to NABH Accredited

With reference on the above subject, approval is hereby granted to Smile ‘N’ Shine Dental Care Centre, 44 Madhya Marg, DLF PH -2, Gurgaon- 122002, empanelled under CGHS Delhi/NCR, as a Non-NABH, Exclusive Dental Clinic to be upgraded as NABH Accredited Exclusive Dental Clinic, w.e.f. 20th May 2022.

The other terms & conditions of empanelment shall remain unchanged
